1.         General

 

1.1       BUNDLED Availability. BUNDLED Pricing is not available for Customer facilities located in the following states: Kentucky, Maine, New Hampshire, North Dakota, South Dakota, Vermont, West Virginia, and Wyoming.

 

1.2       Billing Increments/Call Rounding. U.S. Mainland calls terminating in the U.S. Mainland are billed in an initial increment of 18 seconds, and then in six-second increments thereafter. U.S. Mainland calls terminating internationally are billed in an initial increment of 30 seconds, and then in six-second increments thereafter. If the computed charge includes a fraction of a cent, the fraction is rounded up to the nearest whole cent.

 

1.3       Change in Simultaneous Call Count. For any VoIP Service Type, Customer must maintain its simultaneous call count for at least a 30-day period before requesting a decrease in simultaneous call count.

 

2.         Voice over IP Service Rates and Charges. Rates and charges for each of the VoIP Services types are set forth below. Rates and charges for International calls, certain Local features, directory assistance, and related items are set forth in applicable sections in this Guide.

2.2.      IP Integrated Access Charges. The following charges apply to IP Integrated Access Service. IP Integrated Access Service is available via the A LA CARTE pricing plan with Tiered and Metered pricing options.

 

2.2.1    Tiered Pricing – Simultaneous Calling Capacity Charge. The following charges apply for Simultaneous Calling Capacity (the number of Simultaneous Call Units Customer elects). Minutes cannot be shared between Locations nor can they be rolled over from month to month. Calls to international locations can also be made but are billed at metered rates as set forth separately below in the Rates and Charges section.

 

A.         Monthly Recurring Charge. The MRC applicable per simultaneous calling unit, based on Service Option, will be set forth in Customer’s Agreement for VoIP Service. Each simultaneous calling unit includes unlimited Intra-enterprise calling, unlimited local calling, and an allotment Inter-enterprise outbound VoIP long distance (LD) minutes.

 

B.         Inter-enterprise Overage Charge. Tiered overage charges applicable to minutes in excess of an identified allotment of Inter-enterprise outbound VoIP LD minutes will be set forth in Customer’s Agreement for VoIP Service.

 

2.2.2    Metered Pricing – Simultaneous Calling Capacity Charge. Metered pricing charges apply per simultaneous calling unit multiplied by the number of simultaneous call units Customer selects will be set forth in Customer’s Agreement for VoIP Service. Each such simultaneous calling unit includes unlimited Intra-enterprise VoIP calling and unlimited local calling, while all outbound long distance Inter-enterprise calls will be billed a per-minute charge. Calls to international locations can also be made but are billed at metered rates as set forth in Outbound International Calling, below.

 

2.2.3    Equipment and Access.

 

A.         In addition to the fees set forth above, Customer will pay the applicable fees (not included here) for the gateway, access or transport service (e.g., port and permanent virtual circuit charges), CPE, or any other services or equipment not explicitly described as part of the A LA CARTE pricing. Such fees will be described in the separate contract for the attendant service and/or CPE.

 

B.         A LA CARTE pricing does not include Company’s Internet Dedicated Services or Private IP Service which must be purchased separately by Customer as transport for use with IP Integrated Access.

 

2.2.4    Service Establishment Fee. The following non-recurring charges apply for service establishment.

2.2.5    Feature Profiles. IP Integrated Access sites are provided feature profiles at no charge.

 

2.2.6    Optional Network Features. MRCs applicable to optional network features will be set forth in Customer’s Agreement for VoIP Service. The rates and charges for Redirect to Telephone Number are set forth immediately below:

*  PBX/Key group is defined by groupings of numbers.  When Redirect to Telephone Number (PBX Group Unreachable or Call Forward Unreachable) is provisioned on a PBX/Key group, all numbers in that group will be provisioned and billed with the feature.

 

2.3.      IP Trunking Rates and Charges. The following rates and charges apply to IP Trunking Service. IP Trunking is available via the A LA CARTE pricing model with Tiered and Metered pricing options.

 

2.3.1    Tiered Pricing – Simultaneous Calling Capacity Charge. The following charges apply for Simultaneous Calling Capacity (the number of Simultaneous Call Units Customer elects). Minutes cannot be shared between Locations nor can they be rolled over from month to month. Calls to international locations can also be made but are billed at metered rates as set forth separately below in the Rates and Charges section.

2.3.2    Metered Simultaneous Calling Capacity Charge. Metered pricing charges apply per simultaneous calling unit multiplied by the number of simultaneous call units Customer selects. A minimum of one unit must be purchased for each VoIP IP Trunking hub and remote location. Each such simultaneous calling unit includes unlimited intra-enterprise VoIP calling (VoIP origination and termination) and unlimited local calling, while all outbound long distance (“LD”) inter-enterprise calls (termination is non-VoIP) will be billed a per-minute charge. Calls to international locations can also be made but are billed at metered rates as defined in the Guide.

 

2.3.3    Optional Virtual Fx Service Simultaneous Calling Capacity Charge. The MRC applicable per Simultaneous Call Unit for optional Virtual Fx Inbound Local service will be set forth in Customer’s Agreement for VoIP Service. Each such monthly charge includes the ability to receive one inbound local call at a time. If Customer requires outbound VoIP services at the same physical location where a Virtual Fx-originated call may terminate, separate domestic LD-only VoIP services must be purchased from Company and such services will be offered only on a Tiered or Metered Pricing basis. A separate charge applies for the DID numbers.

 

2.3.4    Equipment and Access.

 

A.         In addition to the fees set forth above, Customer will pay the applicable fees (not included here) for the gateway, access or transport service (e.g., port and permanent virtual circuit charges), CPE, or any other services or equipment not explicitly described as part of the A LA CARTE pricing. Such fees will be described in the separate contract for the attendant service and/or CPE.

 

B.         A LA CARTE pricing does not include Company Internet Dedicated Services or Private IP Service which must be purchased separately by Customer as transport for use with IP Trunking service.

 

2.3.5    Service Establishment Fee. Customer will pay NRCs for service establishment as set forth below.  If Customer purchases a hybrid VoIP service, i.e., IP Trunking together with Hosted IP Centrex, then the Service Establishment Fee for Hosted IP Centrex shall apply.

2.4       Hosted IP Centrex Rates and Charges. The following rates and charges apply to Hosted IP Centrex Service. Hosted IP Centrex is available via the A LA CARTE pricing model with Unlimited, Tiered or Metered pricing options and via the BUNDLED pricing model with Unlimited pricing options. For each Customer location, up to two T-1 lines may be utilized, each of which must be purchased as part of a separate bundle.

 

2.4.1    A LA CARTE Unlimited Pricing – Simultaneous Calling Capacity Charge. The MRC applicable per simultaneous calling unit will be set forth in Customer’s Agreement for VoIP Service. A minimum of one unit must be purchased for each VoIP Hosted IP Centrex location. Each such simultaneous calling unit includes unlimited Intra-enterprise calling, unlimited local calling if the Customer selects the Domestic LD and local service option, unlimited Inter-enterprise LD minutes and LD. Calls to international locations can also be made but are billed at metered rates as set forth in this Guide.

 

Unlimited pricing is available to Customer subject to the following:

 

§  Customer may not utilize VoIP Service in any call center environment or in connection with any similar such application.

§  Customer may not use VoIP Service for telemarketing, fax broadcasting, fax blasting, or continuous or extensive call forwarding.

§  Customer may not aggregate traffic from multiple sites into a single site configured with Hosted IP Centrex VoIP Service.

§  Customer’s design may not be configured with more than 8:1 oversubscription, i.e., no more than eight DIDs per simultaneous call.

§  Customer may not utilize auto-dialers or any similar type of device in connection with VoIP service.

 

2.4.2    A LA CARTE Tiered Pricing – Simultaneous Calling Capacity Charge. The charges applicable to Simultaneous Calling Capacity (the number of Simultaneous Call Units Customer elects) will be set forth in Customer’s Agreement for VoIP Service. A minimum of one unit must be purchased for each VoIP Hosted IP Centrex location. Minutes cannot be shared between Locations nor can they be rolled over from month to month. Calls to international locations can also be made but are billed at metered rates as set forth separately below in the Rates and Charges section.

 

2.4.3    A LA CARTE Metered Pricing. Customer will pay the MRC set forth in its Agreement for VoIP Service per simultaneous calling unit multiplied by the number of simultaneous call units Customer selects. A minimum of one (1) unit must be purchased for each VoIP Hosted IP Centrex location. Each such simultaneous calling unit includes unlimited Intra-enterprise calling and unlimited local calling, while all Inter-enterprise outbound VoIP long distance (LD) will be billed a per-minute charge. Calls to international locations can also be made but are billed at metered rates as defined in the Guide.

 

2.4.5    Equipment and Access.

 

A.         In addition to the fees set forth above, applicable fees (not included here) are charged for the gateway, access or transport service (e.g., port and permanent virtual circuit charges), CPE, or any other services or equipment not explicitly described as part of the A LA CARTE pricing. Such fees are described in the separate contract for the attendant service and/or CPE.

 

B.         A LA CARTE pricing does not include Internet Dedicated Services or Private IP Service which must be purchased separately by Customer as transport for use with Hosted IP Centrex service.

 

2.4.6    BUNDLED Pricing.

 

A.         BUNDLED Base MRC. The base MRC (BUNDLED Base MRC) based on transport option, capacity and router option will be set forth in Customer’s Agreement for VoIP Service. The applicable VoIP Service Equipment is included in the Bundled Base MRC.

 

B.         Composition of Bundled Base MRC. The Bundled Base MRC applies to each bundle at each Customer location and includes the selected BUNDLED transport service (including Shadow T1, if applicable), the applicable VoIP Service Equipment, local access and unlimited outbound domestic long distance and local calling (subject to the number of simultaneous calling units selected). Each bundle is limited to the capacity of the transport service selected (either 768 kbps or 1.5 Mbps) and to the simultaneous calling capacity available and selected for that transport service. The capacity of the Ethernet switch provided via BUNDLED pricing (whether standard or upgraded) is based on the simultaneous calling capacity selected. Additional capacity must be purchased separately.

 

2.4.7    BUNDLED Unlimited Pricing – Simultaneous Calling Capacity Charge. The MRC applicable per simultaneous calling unit will be set forth in Customer’s Agreement for VoIP Service. A minimum of one unit is required. Each such simultaneous calling unit includes unlimited Intra-enterprise calling, unlimited local calling if the Customer selects the Domestic LD and local service option, and unlimited Inter-enterprise LD minutes. Calls to international locations can also be made but are billed at metered rates as set forth in this Guide.

 

Unlimited pricing is available to Customer subject to the restrictions set forth in Section 2.4.1 above:

 

2.4.8    BUNDLED Pricing Limitations - Equipment. Optional IP phones, external firewalls, or any other services or equipment not explicitly described as part of BUNDLED pricing are not included with BUNDLED pricing.

 

2.4.9    Optional Switch Capacity.

 

A.         Extra Analog Gateway Monthly Recurring Charge. The MRC applicable for each additional block of four additional end-users above the simultaneous calling capacity selected under the BUNDLED Base MRC will be set forth in Customer’s Agreement for VoIP Service. The Bundled Base MRC only covers VoIP Service Equipment sufficient for a number of analog gateway ports equal to the simultaneous calling capacity selected.

 

B.         Ethernet Switch for Upgraded Router. If Customer selects one of the BUNDLED Internet Dedicated Services types of BUNDLED transport service which include an upgraded router, Customer may select a Cisco 2950 optional Ethernet switch which supports up to 48 ports for an additional MRC to be set forth in Customer’s Agreement for VoIP Service. Other upgraded Ethernet switches, not described here, are also available pursuant to the separate CPE Service Attachment.

 

2.4.10  BUNDLED Site Activation Fee. The BUNDLED Site Activation Fee applicable for Internet Dedicated transport at each location will be set forth in Customer’s Agreement for VoIP Service.

 

2.4.11  Service Establishment Fee: Customer will pay NRCs for service establishment as set forth below.

2.4.12  Optional Network Features available with A LA CARTE or BUNDLED pricing:  MRCs applicable to certain optional Network Features available with A LA CARTE or BUNDLED pricing will be set forth in Customer’s Agreement for VoIP Service.

 

2.4.13  Optional Network Features available with A LA CARTE or BUNDLED pricing – ICP Platform:  In addition to the MRCs referenced above in Section 2.4.12, MRCs applicable to certain optional Network Features available with A LA CARTE or BUNDLED pricing on the ICP platform will be set forth in Customer’s Agreement for VoIP Service.  The rates and charges for Redirect to Telephone Number are set forth immediately below:

3.      Premium Support Services.

 

3.1       Feature Management. Feature Management will be charged on a pay-as-you-go basis for each type of VoIP Feature Management activity at the following rates:

 

·        User Activity. A charge of $25 per use applies for User Activity. Examples of User Activities include setting up Call Blast or Selective Call Acceptance for a single user.

 

·        Administrator Activity. A charge of $50 per use applies for Administrator Activity. An example of an Administrator Activity is setting up a hunt group or an attendant console.

 

·        Enterprise Activity. A charge of $100 per use applies for Enterprise Activity. An example of an Enterprise Activity is setting up and configuring a three-layer auto attendant.

 

3.2       CPE and LAN Support. CPE and LAN Support activities will be charged at an hourly rate. If the work can be performed remotely, Customer will be charged an hourly rate of $90 per hour with a 30-minute minimum duration.

 

·        During business hours, if the requested support requires dispatch to Customer’s premises, a minimum charge of $265 will apply covering the first two hours of work. After business hours, if the requested support requires dispatch, a minimum charge of $400 will apply covering the first two hours of work. Thereafter, in both cases involving dispatch, an hourly rate of $125 will be charged after the first two hours.

 

·        Company will make a good faith effort to resolve the reported CPE or LAN issue. However, some problems will be the result of causes outside Company’s reasonable control. The hourly charge for CPE and LAN Support is not contingent upon successful resolution of the issue; Customer is obliged to pay the accrued hourly charges for the work performed and invoiced.

 

4.         Outbound International Calling. The following per-minute rates and International Mobile Termination usage surcharges apply to PSTN calls which originate in the United States and terminate in the following international locations. International Mobile Termination usage surcharges apply to calls that terminate via Commercial Mobile Radio Service.

5.         Local Directory Assistance Rates Per Call. Local Directory Assistance is billed on a per-call basis. Customers may obtain two listings per call. If national capabilities are available, two different area codes are allowed. If not, the listings received must be from the same local calling area. Once a caller hears the automated greeting (or the operator), Customer will be billed for the call, even if Customer hangs up before asking for the listing. Additionally, if the operator cannot find a listing or it is "unlisted/not available," Customer will be charged. If an incorrect listing was provided, Customer may call Customer Service to receive a credit for the call. The following Local Directory Assistance charges apply:

6.         Long Distance and/or National Directory Assistance Rate Per Call by State. Call rating is based on where the call originates and what area code was used when 1-NPA-555-1212 was dialed. If these two factors involve two different states, the interstate rate applies. If the call originates in the same state as the area code dialed, the rate per call will be billed as an intrastate Directory Assistance call. For an inquiry out-of-state, Customer may dial any valid area code. Once a caller hears the automated greeting (or the operator), Customer will be billed for the call, even if Customer hangs up before asking for the listing. Additionally, if the operator cannot find a listing or it is "unlisted/not available," Customer will be charged. If an incorrect listing was provided, Customer may call Customer Service to receive a credit for the call.

 

6.1       A $2.75 per-call charge applies to calls to interstate Directory Assistance.

8.         Long Distance Operator Assistance Rates.

 

Long Distance Operator Assistance is billed on a per-minute and per-call basis. The following charges apply to outbound Service calls to United States or International PSTN Operator Assisted calls. For all Verizon VoIP originated calls, an origination credit of $0.045 per minute will apply.  For Verizon VoIP originated calls that, due to operator-assisted completion, terminate over Local Network Connection facilities, a termination credit of $0.01 per minute will apply. For purposes of this Section, a Local Network Connection is defined as a switched connection provided by the Company between a Customer Premises and a Company Point of Presence (POP).

 

8.1       Domestic Calls. For domestic calls made via VoIP (excluding collect calls and calls which are billed by a third party), the following usage charges apply, based on rate period and Mileage Band. For purposes of this Section, the following rate period definitions apply:

 

Day:

 8:00 AM through 4:59 PM, Monday through Friday

Evening:

5:00 PM through 10:59 PM, Monday through Friday

Night & Weekend:

11:00 PM through 7:59 AM

10.       Other Charges.

 

10.1     Direct Inward Dial (DID) Service. Customer may purchase DID service at the rate of $6.25 per month per block of 20 DID number plus an installation charge of $5 per block of 20 DID numbers.

 

10.2     Dispatch Charge. A $265 charge applies for each occasion in which Company dispatches a technician to make a Customer-requested change or adjustment in VoIP, unless required to accommodate the addition of new services. Subject to change as posted in the Guide.

 

10.3     Expedited Provisioning/Installation. A non-recurring charge applies per circuit to expedited provisioning of Internet Dedicated transport, whether the circuit is provisioned by Company or by another local exchange carrier. A non-recurring charge applies per location for any location for which Customer-requested expedited installation. These non-recurring charges are found in the following list of Administrative Non-Recurring Charges:

10.4     Maintenance Charges. If repair and/or replacement of VoIP Service Equipment is required because of damage caused by Customer’s negligence or willful misconduct, Customer will be charged time at a rate of $125 per hour to repair or replace, as appropriate, the VoIP Service Equipment, and Customer will be charged the replacement cost of the VoIP Service Equipment, if replacement is required.

 

10.5     Network Application Fees. Network application fees are charged for domain name, mail, new services, and other network applications. See the Network Application Fee Schedule at www.verizonbusiness.com/terms.

 

10.6     On-Site Training. On-site training is available pursuant to a separate site service attachment at the rates set forth for VoIP service.

 

10.7     Return Charges. If a Customer fails to return VoIP Service Equipment, or any portion thereof, as set forth in the Terms and Conditions section below, the Customer will be billed and required to pay an amount equal to: (i) the VoIP Service Equipment manufacturer's list price in effect for at the time of expiration or termination of the rental period for the VoIP Service Equipment; and, (ii) any other associated charges incurred by the Company.
