Six Month Trial Promotion

Offering:  Upon the Customer’s request to the Company, the Company will waive the underutilization and early termination charges set forth in Special Customer Arrangements (SCA) Guide Types 1, 6, 7, 8 and 9 if the Company receives, within 180 days of the date of the Customer’s enrollment in the SCA Type, written notification from the Customer that it wishes to discontinue service under the SCA during that 180-day period.  Customers who terminate all service under this SCA Type will be billed and required to repay on a pro rata basis all credits received and charges waived (excluding installation charge credits and waivers) under the SCA.

Eligibility:  The Customer must

enroll in this promotion between February 1, 2003 and April 30, 2003;

enter into an SCA Guide Type 1, 6, 7, 8 or 9 between February 1, 2003 and April 30, 2003;

not have received Company service during the 90-day period preceding SCA enrollment; and,

represent to the Company’s satisfaction, as determined with the Company’s sole discretion, that it will not enter into an SCA without further inducement from the Company.
