LD Voice – Interstate 30 Promotion

 

Offer:  Customers contracting for Long Distance Voice Services for the first time will receive a credit in each 12-month period (not to exceed a total of three credits) following the “Effective Date” of the Verizon Business Service Agreement (Agreement) or amendment adding Long Distance Voice Services.  The credit will be provided at the time specified below, and will be applied to Long Distance Voice Services usage charges at standard Guide rates.  The amount of the credit is equal to Customer’s average monthly interstate inbound and outbound long distance service per-minute usage charges for the relevant 3-month period.   The table below sets out, for each 12-month period, the month in which the credit is provided, and the 3-month period used for calculating the credit.  The start date for determining each of these periods is the Effective Date of the Agreement or amendment adding Long Distance Voice Service:

Eligibility:  The Customer must:

 

enroll in this promotion between August 1, 2006 and September 30, 2008;

 

contract for Long Distance Voice Services under this promotion and an Agreement with a minimum one-year Term commitment remaining; and

 

request installation of new Long Distance Voice Services by November 30, 2008 to occur no later than December 31, 2008.

 

Other Conditions:   Customer may not receive the benefits of Product Package Guide Type 1.

 

Notwithstanding the forgoing, any credits not received by the time Customer terminates Long Distance Voice Services are forfeited and not owed.
