IV.1.2.2.D Pre-Verizon Business Services I Charges:

The following monthly recurring per-PVC charges apply to PVCs between (i) ports in Guam, and (ii) ports in the following locations, which PVCs are provided under Company contracts, or contract amendments, whose rates are first effective after March 31, 2003 and are not provided under a Verizon Business Services I or subsequent pricing plan:
2. The following monthly recurring per-PVC charges apply to PVCs between (i) ports in Guam, and (ii) ports in the following locations, which PVCs are provided under Company contracts, or contract amendments, whose rates are first effective after October 31, 2002 and prior to April 1, 2003:

The following monthly recurring per-PVC charges apply for PVCs associated with ports between Guam and the following locations that are installed, or provided under Company contracts signed or renewed, after February 6, 2002 but not to PVCs provided under Company contracts, or contract amendments, whose rates are first effective after October 31, 2002:
4. The following monthly recurring per-PVC charges apply for PVCs associated with ports installed prior to February 7, 2002 between the Guam and locations in the following regions. For purposes of this Section, the following locations are defined as part of the following regions: Region List.
