Verizon Business Services II Flexible T1 Plus Promotion
Offering: In each monthly period of Customers Product Package term of service, Customer will receive the following benefits in lieu of the benefits set forth under Product Packages Guide Types 18, 19, 20 and 21 for Eligible Interstate Service usage.
The following monthly recurring charges apply to Customers Eligible Interstate Service usage, based on the tier, and the maximum minutes of use for each tier for both: 1) Eligible Interstate Service usage, and 2) Eligible Intrastate Service usage per Flexible T1 to which Customer subscribes under Companion Local Service (Eligible Intrastate Flexible T1 Service):

If Customers combined usage of Eligible Interstate Service and Eligible Intrastate Flexible T1 Service exceeds the maximum minutes of use per tier, Customer will be billed $0.35 per-minute for Customers Eligible Interstate Service.
Company will waive installation charges and start-up fees for Frame Relay Services, Internet Dedicated Services at T-1 bandwidth, Private IP Service transported via a Flexible T1 to which Customer subscribes under Companion Local Service.
Eligibility: Customer:
must enroll prior to March 31, 2007;
must be either a New Customer or a Renewing Customer;
must designate the Company as its exchange service, intrastate and interstate telecommunications service carrier;
must subscribe to Company service under Product Packages Guide Types 18, 19, 20 or 21;
must subscribe to Verizon Business Service II Flexible T1 exchange service as described in the appropriate Company exchange service tariff or other appropriate governing document ("Companion Local Service") concurrent with enrollment in this promotion;
must transport Frame Relay Services, Internet Dedicated Services at T-1 bandwidth, Private IP Service or Company exchange service on at least twelve channels of each Flexible T1 to which Customer subscribes under Companion Local Service;
must place order on or before April 30, 2007 for installation of all service which receives the benefits of this promotion; and,
may not receive: any discounts other than discounts under Customers Product Package; or, the benefits of any promotional offering or Optional Calling Plan.
Definitions: For the purposes of this offering, the following definitions apply:
Eligible Intrastate Service: Verizon Business Service II Long Distance Voice Services Outbound Service (excluding Card) usage that originates and terminates in one state, excluding exchange service, as set forth in the Companion Intrastate Tariffs or other appropriate governing documents.
Eligible Interstate Service: Verizon Business Service II Long Distance Voice Services Feature Outbound Service (excluding Card) usage that originates in the U.S. Mainland and terminates in the U.S. Mainland, Alaska, Hawaii, Puerto Rico, the U.S Virgin Islands, Guam, and CNMI.
New Customer: Customer who, at the time of subscription to this plan, is not receiving service under this Guide and the tariff or other appropriate governing document containing the Companion Local Service.
Renewing Customer: Customer who, at the time of subscription to this plan, is receiving Company service under a term of service commitment with fewer than 3 monthly periods remaining in the term and enters into a new term of service commitment for Company service.
Other Conditions:
If Customer disconnects Companion Local Service and Companion Intrastate prior to expiration of Customers Product Package term of service, continues to maintain a Company account, and does not subscribe to another service offering under the Guide, Customer will be automatically subscribed to Verizon Business Service II Long Distance Voice Services.
